With receipt of this pre-registration form and a US$200 (US$215 if paying with a credit card) non-refundable deposit by 25 March, 2011, you will reserve your spot for the 2012 event. If we do not receive your pre-registration deposit by that date,your spot will be offered to the next person on the waiting list.
